Spasmodic Pain: Causes, Mechanism & Treatment

Spasmodic pain is also known as smooth muscle pain or visceral pain. It occurs due to excessive contraction of smooth muscles, especially in hollow organs of the body.

📌 Introduction

When smooth muscles of hollow visceral organs contract abnormally, they stimulate pain fibers and cause discomfort. This type of pain is commonly called colic.

  • G.I. Colic: Stomach & intestine
  • Renal Colic: Kidney
  • Biliary Colic: Bile duct
  • Ureteric Colic: Ureters
  • Uterine Colic: Uterus

📍 Site of Occurrence

Smooth muscles of hollow visceral organs.

Mechanical in nature.

⚙️ Mechanism of Spasmodic Pain

  1. Mechanical stimulus activates parasympathetic nerves
  2. Increased release of Acetylcholine (ACh)
  3. ACh binds with M1 muscarinic receptors
  4. Leads to excessive smooth muscle contraction
  5. Results in spasm (colic) and pain

Important Cycle: Anxiety → Spasm → Pain → More Anxiety

This is called the anxiety–spasm–pain vicious cycle.

💊 Treatment of Spasmodic Pain

Treatment mainly focuses on:

  • Preventing the root cause (inhibition of Acetylcholine)
  • Reducing anxiety (psychosomatic factor)

1️⃣ Anticholinergic (Antimuscarinic / Antispasmodic) Drugs

These drugs block the action of Acetylcholine by binding to M1 receptors, preventing muscle contraction.

Mechanism: Competitive inhibition of Acetylcholine

Examples:

  • Dicyclomine Hydrochloride (most preferred)
  • Atropine Sulphate
  • Hyoscine Butylbromide
  • Flavoxate Hydrochloride
  • Oxybutynin Hydrochloride

Note: Atropine is less used nowadays due to side effects as it crosses the blood-brain barrier.

Why Dicyclomine is Preferred?

  • Water-soluble
  • Does not cross blood-brain barrier
  • Fewer side effects
  • Strong antispasmodic action

2️⃣ Anxiolytic (Anti-Anxiety) Drugs

These drugs help break the anxiety-spasm-pain cycle.

Examples:

  • Diazepam
  • Lorazepam
  • Oxazepam
  • Alprazolam

Importance: Even if muscle contraction is controlled, anxiety can still trigger recurrence. Hence, anxiolytics are essential.
